Application Requirements:
  • Company Your company name, contact information, TIN (Taxpayer Identification Number) and E&O insurance information.
  • Primary Agent Your primary agent's email address, contact information and license data (including digital copies of their license) for the states they are authorized to work in.
  • Coverage Area The counties that the primary agent will cover.
